NICHQ teacher-completed ADHD symptom and academic performance assessment. AAP-endorsed; screens for inattention, hyperactivity/impulsivity, ODD/conduct, and anxiety in school setting.

Instructions

Please rate each behavior on a scale of 0–3, where 0 = Never, 1 = Occasionally, 2 = Often, 3 = Very Often. Behaviors occurring 2 or 3 times per week count as symptoms. Part A: Behavior items in school. Part B: Academic performance (1 = Excellent to 5 = Problematic).
