Instructions

Please rate each behavior on a scale of 0–3, where 0 = Never, 1 = Occasionally, 2 = Often, 3 = Very Often. Behaviors occurring 2 or 3 times per week count as symptoms. Part A: Behavior items. Part B: Performance (1 = Excellent to 5 = Problematic).
